Adult Practice Reviews

There is an obligation for all agencies involved in the care, support and protection of adults at risk to ensure that the highest possible standards of care, support and protection are provided and maintained at all times. Part of this obligation is a requirement to learn from mistakes, especially those resulting in the death or serious injury of an adult at risk.

 

In accordance with the Social Services and Well Being (Wales) Act, the Cwm Taf Safeguarding Adults Board have a responsibility to establish an Adult Practice Review when the criteria requiring one are met.   

 

Professionals who wish to refer a case to be considered for an Adult Practice Review are asked to complete the APR/CPR Practice Review Form and forward it to the Safeguarding Business Unit.
